What we collect

When you submit the eligibility form, we collect:

• Your name, phone number, and email address
• Your state and insurance type
• Plan name (optional)
• Whether you have a doctor who can write a prescription
• The page you submitted from
• Your consent choices

We do not collect Social Security numbers, full date of birth, or financial information through this site.

What we do with it

• Match you with a Medicare-enrolled DME supplier. We forward your information to a supplier who can confirm your coverage and start your order.
• Send you the answer you asked for. If you opted into email follow-up, we may email coverage information related to your request.
• Improve the site.We use anonymous, aggregated data to understand which pages help and which don't.

What we don't do

• We don't sell your information.
• We don't share your information with insurance brokers, Medicare Advantage plans, or marketing companies.
• We don't use dark patterns. The boxes you check or don't check are honored. You can opt out at any time.

Calls and texts (TCPA)

By providing your phone number and checking the consent box, you agree to receive calls and texts from Covered. and a Medicare-enrolled DME supplier related to your request. Consent to receive automated calls/texts is not a condition of receiving service. Standard message and data rates may apply. You can opt out of texts by replying STOP. You can opt out of calls by saying so on any call.

Call recording and AI voice

Calls related to your eligibility request may be recorded for quality and verification. AI voice assistants may be used during these calls. You will be notified at the start of any recorded or AI-assisted call. You can opt out at that time.

Cookies

We use minimal first-party cookies for site performance. We do not run third-party advertising trackers. We may use a privacy-respecting analytics provider that does not store personal information.

Your choices

• You can request a copy of the information we have about you.
• You can request that we delete it.
• You can opt out of communications at any time.

Covered. is not a covered entity under HIPAA. Information you share with the DME supplier we connect you with is governed by that supplier's HIPAA-compliant policies.
